I’ve had a similar experience and thank goodness no one was hurt. My experience was with a new to me boat, a few months post survey, which included an inspection of the system. The failure occurred while at the dock with the crane secured. There was a loud bang. Upon inspection the SS cable had broken and the weight had fallen on to the deck.
Upon inspection the cable was fine EXCEPT for the area that was inside the weight. My estimate is that the SS broke down due to a wet environment with a reduced o2 level
 
I have a Nick Jackson davit on my boat (1000 lb.) lifting a 12' Novurania with 25 hp. 4 stroke. Have had it for about 3 years....works great. Rides on the starboard side of 46' GB Classic. Great crane.
