If I drop anchor in waters off an island, for one ro two days rest, without
going on that country's land, do I still have to check in to customs at that
country>
 
Yes. If you remain underway its not a problem, but as soon as your anchor hits the bottom, you have arrived

Yes. For example, Canada states:

If you enter Canadian waters, you are not required to present yourself and report your goods to the CBSA if you:

do not land on Canadian soil and do not anchor, moor or make contact with another conveyance while in Canadian waters

do not embark or disembark people or goods in Canada

If at any point during the transit you come to port, anchor, moor or make contact with another vessel, you must report to the CBSA immediately

I don’t know where you are geographically but after 47 countries/territories by boat, French Guiana was the only country that didn’t care. In the Caribbean, you may get away it because of the charter fleet but…

We did an over night passage from Trinidad to Tobago and checked in at 8am. The officials asked about the boat anchored near us. They were catching up on their sleep and were fined for it. :)
