Changing all thru hull fittings, should I use 4200, 5200 or what else?

If you're never taking them out again, hard to beat 5200, IMO.

Either. If the yard ever has to replace them they will put a torch on the fitting to soften the 4200/5200 so either will work. 5200 is more for adhesion which in a thru hull you don't need, so my preference would be 4200.

I can see using 5200 or 4200 for thru hull fittings but what should I use for struts as they may need to come off.

Any metal fitting bedded with 5200 is pretty easy to remove just by heating it.
I always used 5200 for any underwater fitting and never regretted it.

Be careful getting 4200 on threads that you don't want it on, very sticky and it makes it hard to turn.

5200 for sure. Never had an issue removing' as stated a little heat will do it.

The USCG practice when inspecting vessels that can carry over 6 is to ask for a thru hull or two be removed for inspection.

Of course this is not required for a non inspected vessel, but if the USCG has found it to be a worthwhile practice , sealing thru hulls should be done so the can easily be inspected. Not installed with glue, but with sealant.

4200 and the proper removal tool will make inspection a few min job , should you desire to look.
