What is the purpose of the sand shoe. It does not seem to carry any load. Does it just protect the prop?

Where the rudder shaft enters the sand shoe there is approx 1/4 inch of play
Should there be a bearing or sleeve to take up the space or is this the way it was built
The sand shoe does not seem to support the rudder
 
While I can't speak specifically to Mainships, the shoe is designed to help with horizontal loads and not really vertical loads. The one on my charter boat became worn. Had it bored out and pressed a section of cutlass bearing into it to make it snug again. The bearing lasts 4 or 5 seasons and then I replace it.

I think I remember seeing it a brass ring or spacer in that space. The boat's in the water so I can't tell at this time.
