In photos of larger yachts, such as the Fleming 55, I see a pair of throttle handles to the right of the ship's wheel, with red knobs on top, and to the left of the wheel, a pair of what I believe are called gear handles, that look identical to the throttles, except they have black knobs on top.

What is a gear handle for, forward and reverse? At the throttle handles just zero to full, not full back to full forward?

correct...red adjusts RPM....black is fwd, neutral and reverse.

And let me tell you - after coming from 40yrs of single outboard powered boats, my twin engine Riv (with the throttle/gears as you describe) was a bit of getting used to in close quarters of the marina (another new experience that came with the new boat), I can tell you.
