Moral of the story:
1. UPDATE YOUR CHARTS, no matter which charts you are using
2. LOOK UP!! Observe your surroundings. No chart can never be 100% up to date all the time so use some common sense and look up.
3. It's a good idea to have multiple charts onboard and when in doubt, don't take the route.
