Form the USCG:

The North and South Jetties at the Columbia River Entrance have suffered severe deterioration and may no longer be correctly represented upon the nautical charts of the area. Mariners should use caution when transiting in the vicinity of the jetties and the river entrance. The USACE has scheduled critical repairs of the North Jetty for June 2015 through Oct 2016, and rehabilitation of the South and North Jetties scheduled for the years 2016 through 2019. More information may be found at the USACE Portland District web site:

Do yourself a favor and always transient the mouth on an in coming tide and/or during calm waters. The out going current flows in excess of six knots.
