If it is a head gasket the only way to know is to find out if you have exhaust gas in your coolant. Napa sells a kit to test for this.
Do you plan on doing the work yourself?
if you do it yourself you have to pull the head off.
Not hard to do. Just heavy. Each head weighs about 120 lbs.
if you take the head off. Turn it with the valves facing up and spray it heavy with break clean then take a vacuum cleaner on blow and blow air into the intake you will see bubbles around the valve seat .If you have a bad valve it will blow by the valve seat. no bubbles no bad valve seat.

It is just nuts and bolts and a bit of heavy lifting. the hardest part was getting the injectors out. a ladys food works. Take pictures of every thing you do so you can put it all back together.

    I hope this helps.

Saw your post about 3208na. I have a californian 43 with twin 3208na. Last week one started making loud fluttering sound out of intake. Could also hear sound in exhaust tube. Consensus is valve or head gasket. Any idea on repair cost? Engines have 2500hr. Difficulty of repair?

    Thanks. Steve in south Florida
